State of Indiana County of Martin (no date given)
Pension claim no. 327216 of Julia A Bowman, mother of Pleasant Bowman deceased late of Co K 49th Ind Inf vol.

Personally came before me a notary public in and for the aforesaid county and state William J Thomas a citizen of Keck's Church in the county of Martin and state of Indiana well known to me to be reputable and entitles to credit, and who being duly sworn, declares in relation to the aforesaid case as follows:

That he is a regular practicing physician of 10 years standing and has given medical advice and treatment to William H Bowman as follows:

I first treated him July 30 1881 until August the 12, 1881. When he had an attack of Cerebro Spinal Meningitis and pneumonia. My next treatment for him was April 25, 1882, he then had an attack of remitent fever and albuminuria.

The next treatment that I was called to treat him was Feb 10 1883 which was for another attack of remitent fever and lumbago. I was called to treat him again Dec 10 1884 for intermitent fever. I see William Bowman very frequently for the last five years on an average of once a week and I believe that he is a constant sufferer of lumbago judging from the many times that I have heard him complaining of said disease.

I have no interest in the prosecution of this claim.
