Note: We have a death record from the Oregon State Hospital located in Portland, OR. A William Andis (born Indiana) was admitted 26 Dec 1911 for Dementia. He died of apoplexy on 16 Oct 1912 at the age of 72. He was buried in an unknown cemetery in Portland, OR.
